人工晶体植入术后继发性青光眼的手术时机
【摘要】 目的探讨人工晶体植入术后继发性青光眼(implantation secondary glaucoma,IOL)的手术时机。方法收集自2010年12月--2015年12月收治的22例人工晶体植入术后IOL患者,根据手术时机选择的不同将22例患者(22眼)分为观察组(n=12)和对照组(n=10),观察组患者在眼内压30~50 mm Hg(1 mm Hg=0.133 k Pa),药物治疗无效3~5 d时的难治性青光眼行小梁联合虹膜根切术进行治疗,对照组患者在眼内压超过50 mm Hg,药物治疗无效超过7 d的难治性青光眼再行上述手术治疗,随访并记录两组患者术后1周、1个月、3个月的眼内压,并行统计学对比。结果两组患者手术过程均顺利,观察组患者术后1周眼内压(18.2±9.0)mm Hg,1个月眼内压(14.3±8.2)mm Hg,3个月眼内压(15.7±8.9)mm Hg,与对照组相比,术后1周、1个月眼内压,差异存在统计学意义(均P&0.05);术后3个月两组患者平均眼内压较为接近,差异无统计学意义(P&0.05)。结论小梁联合虹膜根切术是治疗人工晶体植入术后IOL的有效术式,在其发病早期,眼内压30~50 mm Hg时行此手术治疗,能缩短眼内压恢复正常的时间,改善患者预后,提高生活质量,值得临床推广应用。
